Although I have a good idea of what you require, the one thing that has stumped me is that you're asking for a dedicated server solution when your requirements to me scream colocation!
Dedicated servers have a large background in being on non-enterprise hardware that is not tailored towards production services. Although we could provide you a solution in this area I think if you're big enough to have requirements as outlined above and knowledgeable enough to run them all then you should really by now have it on your own hardware.
In my opinion the key advantages of this are;
1) You can change the hardware around as much as possible and add / remove peripherals as the server is yours.
2) If you decide you don't like your provider you can move away without having to completely reprovision your server (just simple IP changes)
3) You're only paying colocation costs, not paying off someones server over a few months.
4) You know what you are getting (enterprise grade!) as opposed to whiteboxes that have good specifications but aren't made for constant use.
Apart from that, Micron21 has 24/7 support, we run and own our datacentre, we can organise IPs no problem, pride ourselves on custom solutions and have very competitive services with or without support.
